Motor Insurance

Step 1

a) Exchange Particulars of Parties Involved in the Accident and take note of the vehicle numbers
b) Take photos of the scene of the accident and the damages to all vehicles

Step 2

Call Your Insurer's Hotline for a Tow-Truck or further advice on the accident

Step 3

Report within 24 Hours or by the next working day

Step 4

If there are injured parties in an accident, call for medical assistance and the police immediately.

Step 5

Make a Traffic Police report as soon as possible or within 24 hours of the accident if the accident involves:
- Fatality;
- Damage to government property;
- Foreign vehicle;
- Pedestrian or cyclist;
- Hit-and-run case; or
- Injury cases where

i. At least one person involved in the accident was taken to hospital from the accident scene by an ambulance/ self conveyed; or

ii. Any party involved in the accident was injured and obtained outpatient medical leave for 3 days or more.

Step 6

Please note that after filing a Traffic Police report, you are also required to report to your insurer within 24 working hours or by the next working day.

Do not move your vehicle before taking photographs of the accident scene.

Maid Insurance

Step 1: Prepare required documents

1. Original final hospital/medical bills and receipts
- For Hospital Benefit rider, you only need to provide copies of the final hospital/medical bills and receipts.
2. Hospital discharge summary
3. Medical reports, if any
4. If you have submitted a claim to other policies/insurers/your employers/any other third parties who have reimbursed your bills, please submit copies of the following documents:
- Reimbursement letter
- Discharge voucher
- Payslip reflecting the medical expense deduction
5. A clear copy of policyholder and insured’s NRIC (both sides)

Step 2: Complete relevant claim form

Please print and complete the form required for your plan.

Step 3: Submit completed claim form and required documents

Home Insurance

Step 1: Complete claim form

Death claim

-­ Death certificate, autopsy report, coroner’s findings
-­ Proof of relationship between the insured and the claimant. Examples include copy of marriage certificate, birth certificate etc.
-­ Police report, if applicable Permanent disability claim
-­ Medical report Medical expenses claim
-­ Original medical bills and receipts
-­ If original documents have been submitted to another insurer/employer, please provide:
o Photocopy of the bills
o Reimbursement letter/discharge vouchers from the other insurer/employer indicating the amount paid by them

Hospital & surgical expenses/Wages compensation claim

-­ Original in-­patient bills and receipts
-­ In-patient discharge summary/medical report Expenses claim for termination of maid’s contract
-­ Death certificate

-­ Doctor’s note confirming permanent disability or any serious work-­impairment (e.g. serious sickness/injury)
-­ Invoices of expenses to be claimed

Business Insurance

Work Injury Compensation Claims

Step 1: Complete claim form
Please submit an online report of the incident to MOM (you can refer to MOM website on the procedure (www.mom.gov.sg). Please also print and fill up the Work Injury Compensation Claim Form.
 

Step 2: Prepare required documents
Please note that if necessary, the insurer may request additional documents. For the full list of required supporting documents to be submitted, please refer to the claim form.

-­ Police report and/or report(s) lodged with Ministry of Manpower where applicable
-­ Original medical bills and receipts
-­ Medical leave certificates
-­ Work permit (for foreign workers)
-­ Payslip/salary voucher for the 12 months immediately before the date of incident
-­ Proof of project/works if you are a sub-­‐contractor (e.g. contracts, quotations and invoices)

-­ Copy of police report if crime is involved
